Receiving Treatment at the Hospital

Please bring a referral letter from another medical institution with you.
The hospital is designated as a community medical support hospital with more than 500 beds. Therefore, if no referral letter is provided, a 7,700 yen (tax inclusive) selected medical care fee will be applicable in addition to the treatment fee and the fee for the first medical examination etc.
Even if you already have a patient registration card, in cases where, for example, post-recovery, you are now seeking treatment for a different condition, or where you wish to receive treatment from a dentist for the first time (this includes treatment with a different department while receiving treatment from the dentist), you are required to pay the selected medical care fee unless a referral letter is otherwise provided to us.
Also in the case of follow-up treatment, where you choose to receive treatment from us even after a referral has been issued to a different medical provider, you are required to pay a 3,300 yen (tax inclusive) selected medical care fee.
Your understanding regarding this matter is appreciated.

 

Treatment Reception and Start Times

Treatment Reception Hours
First Treatment: 8:30 a.m. to 11:00 a.m.
Follow-up Treatment: 8:20 a.m. to 11:00 a.m. (except for appointments)
Non-treatment Days: Saturdays, Sundays, public holidays, and New Year's holidays
(December 29 to January 3)

Treatment starts at 9:00 a.m.
As our car park gets very busy, especially in the morning, please use public transportation as much as possible.
Should you choose to use the car park, please arrange to arrive early.

 

First Treatment (For Those with a Referral Letter)

◆ Please fill out the necessary information in the medical treatment application form and submit it to the designated counter for referred patients together with the referral letter (plus any other information such as images etc., if applicable), and the health insurance card.

◆ After completing registration at the designated counter for referred patients, please register again with the applicable clinical department’s reception (as for the second floor, the receptions are at each block). After this, please wait in the area in front of the relevant examination room. Please keep your token until you complete payment as we call you in to the examination room by displaying the number on your token on the display board in front of the examination room.

 

First Treatment (For Those without a Referral Letter)

◆ Please fill out the necessary information in the medical treatment application form and submit it to the first treatment counter together with your health insurance card.

◆ After completing registration at the first treatment counter, please register again with the applicable clinical department’s reception (as for the second floor, the receptions are at each block). After this, please wait in the area in front of the relevant examination room. Follow-Up Treatment

Please register using the follow-up treatment registration machine. Even if you already have a patient registration card, if you are visiting a particular clinical department for the first time, please proceed with your registration at the first treatment counter. However, if you have a referral letter, please complete your registration at the designated counter for referred patients.

◆ On the day of the examination, please register using the follow-up treatment registration machine regardless of whether you have an appointment or not.

◆ Not only for examinations, but also for tests and x-rays, please register using the follow-up treatment registration machine in the same way.

◆ For anyone who has not received treatment from any of our clinical departments for six months or more, we are unable to process registration using the follow-up treatment registration machine. Instead, please visit the follow up treatment counter to complete registration. When you do so, please submit your health insurance card together with your patient registration card.

◆ If you have not visited the particular clinical department before, please register at the first treatment counter. (Please fill out the necessary information in the medical treatment application form and submit it at the first treatment counter together with your health insurance card. If you have a referral letter, register at the designated counter for referred patients.)

◆ Once the follow-up treatment registration is done, please hand in your token to the reception indicated on it. After handing in the token, you will be guided via the display board in front of the examination room. Also, when you return back to the clinical department after completing the test, x-ray etc., please register again (confirm your arrival) with the same reception where you handed in your token initially.

◆ In cases where you choose to receive treatment from us even after a referral has been issued to a different medical provider, you are required to pay the selected medical care fee.

◆ Please understand that the order in which patients are seen may change according to the progress of tests etc., and/or any emergency patient treatment etc.

 

Outpatients Treatment Fees

◆ After the examination, in the examination room or waiting room, you will receive a file containing your provisional outpatient schedule, an external prescription, and an appointment slip (if applicable). Please hand in the file to the accounting counter located on the first floor of the outpatients ward.

◆ In general, payments are processed through the automatic medical expenses accounting machine. A member of the accounting counter staff will issue you with a numbered token for the accounting process. Please proceed to the automatic medical expenses accounting machine and settle your account once your number is displayed on the accounting information display board.

◆ If you have an in-house prescription, an exchange coupon for your medicine is attached to your payment receipt.

◆ For payment, we accept cash, various credit cards and debit cards. For more details, please inquire at the accounting counter.
Security deposits

◆ Anyone who receives dental implant treatment will be required to pay a security deposit at the time of giving us consent regarding the treatment. The security deposit received will be deducted from the bill once the implant treatment is completed. Please submit the deposit receipt which we issue at the time of receiving the deposit to the accounting counter when the implant treatment is completed.

* Two teeth 420,000 yen
* Additional teeth after the second 160,000 yen/tooth

◆ Should we be unable to calculate the treatment fee due to a system failure or a power cut etc., we may ask you to pay a deposit of 5,000 yen at the time of settling the account. In such cases we will issue you with a deposit receipt. Please submit the receipt to the accounting counter next time you visit the hospital.

 

Medicine

◆ If you are currently taking medicine, please provide us, at the time you receive treatment, with information which explains what medicine you are taking, such as a medicine logbook. This may be required during the examination.

◆ For anyone who receives an external prescription
Please collect your medicine from an external pharmacy. (Payment is required separately.)
We provide a fax service to external pharmacies from our external prescription corner.

◆ For anyone who receives an in-house prescription
The medicine exchange coupon number will be displayed on the pharmacy’s electronic notice board.
Once your number is on display, please receive your medicine from the pharmacy counter.

 

Patient Registration Card

◆ The card is valid at all clinical departments in the hospital. Please make sure to bring it when you receive treatment.

◆ Should the card be lost, please report this to the follow-up treatment counter. (Reissuance fee applies)

 

Health Insurance Card

◆ Please show your health insurance card every month at the health insurance card confirmation counter or the accounting counter.

◆ Please be aware that, should the health insurance card not be shown, we will be unable to proceed with health insurance treatment.

◆ Should any details on your health insurance card change, please advise the health insurance card confirmation counter or calculation counter.

 

Requests to All

◆ Since May 1, 2006, all buildings and hospital premises are smoke-free. We appreciate your understanding and support in this matter.

◆ Please do not use any portable communication devices such as mobile phones and smart phones in prohibited areas (examination rooms, dispensaries, blood sampling rooms, treatment rooms, testing rooms, operation rooms, inside elevators and on escalators). Also, please put your phone on silent mode inside the hospital.

◆ Always be aware of your belongings and valuables.

◆ Please take care of your personal belongings. Please note that the hospital will not be held responsible for any damage to or loss of false teeth or hearing aids.
